1. Instructions
When you are ready to sign, click on step 2 and fill out the form. We will then take this signature and apply it to both the Board Resolution and Memorandum of Understanding portion of this document. Please ensure that you have proper authority to sign this proposal. Once you have signed, you will be directed to a page where you can view, download/print, and store the proposal for your records. We will also use our copy to authorize the beginning of stages of starting your new retirement plan.
Your signature will be used on:

2. The Board Resolution (See Page 14)

These documents, once signed, will be reviewed by our record-keeping department. Should there be questions, the document may be voided and we will reach out to you to revisit the signature process.
